Integrated care is multidimensional and multifaceted, encompassing a common concept of integrated care, identifying the content and goals of the integration of healthcare and social services, identifying the need for integrated care and influencing personal care. The organisation of integrated services that meet the needs of the patient poses new, complex challenges: finding the right mechanisms for organization and funding of integrated services and overcoming institutional barriers between healthcare and social assistance. The dissertation “Model for the Organization of Integrated Healthcare and Social Services in Lithuania” aims to better understand the aspects of the development of the integrated system of healthcare and social services as a managerial process, to identify the need for these services and contradictions with integration, to present the principles and measures of service organization that reduce the fragmentation of care services, improve the availability and quality of services and ensure the continuity of services at the outpatient level. The model for integrated healthcare and social services detailed in the dissertation focuses on the integration of healthcare and social services at the outpatient level in order to ensure long-term care services in Lithuania in the context of an ageing population.
