Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for patients with colorectal carcinoma: patient selection and improving outcomes
Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for patients with colorectal carcinoma: Patient selection and improving outcomes In the Netherlands, colorectal cancer is one of the most common types of cancer in women as well as in men. Every year, around 4500 people die because of this disease. Treatment of colorectal cancer consists of a combination of surgery, medication and in some cases radiotherapy. In case of peritoneal metastases, treatment has been difficult, therefore survival of these patients is limited. In the Netherlands, a selected group of patients with peritoneal metastases can be treated with a CRS+HIPEC procedure. During this procedure all macroscopically visible tumour is removed and heated chemotherapy (oxaliplatin or mitomycin) is flushed through he peritoneal cavity during 1.5 hours. With this treatment a 5-years survival of 35-43% has been reached. Only patients that have no other metastases than peritoneal metastases are eligible for CRS+HIPEC. In consequence of the complexity of the procedure many patients experience complications. Therefore, it is important to select those patients that will benefit from the procedure while experiencing a minimum of complications. In this thesis we focused on a more effective patient selection, optimisation of the treatment and the prediction of the quality of life of patients after a CRS+HIPEC procedure. The aim is to find the best treatment for the individual patient, to discover possible predictors of complications and ultimately improve the quality of life of patients.
