This dissertation reports on multi-year research into the relationship between accountability and learning in case consultation meetings for complex cases. Serious incidents can be prevented there, such as the murders of ‘the Maas girl’ and Els Borst. The outcome of the research is not to opt for accountability or learning, nor to divide the attention between the two concepts, but rather to consciously shape a relationship between accountability and learning in partnerships so that they reinforce each other. In this way, the chance of dramatic incidents occurring again in the future can be further reduced.
